The Ambulatory Practice of the Future (APF) is an innovative primary care clinic focused on comprehensive, proactive population management by empowering patients to engage in their own care. The APF care model has been designed with the patient at the center of a collaborative, interdisciplinary care team focused on the patient’s health goals and priorities. To meet the patient’s needs the model relies heavily on the use of technology including data capture, communication, and tools.

Technology Adoption, Challenges, and Opportunities

The Ambulatory Practice of the Future (APF) is an innovative primary care clinic focused on comprehensive, proactive population management by empowering patients to engage in their own care. Join us at Massachusetts General Hospital’s Ambulatory Practice of the Future to learn about the path they have taken to help cross the chasm to better care and lower costs.  Dr. David Judge, Medical Director of APF, will provide an overview and tour of the facility.  He’ll discuss their roadmap including the technologies they have implemented and the technologies they are on the hunt for next.  

MassTLC brings together the people that can solve the healthcare dilemma of lowering costs while improving care.  It provides the forums for gathering knowledge to accelerate change and move forward with the right roadmap.  Join us as a member of MassTLC for an insightful evening, which will kick-off a year of facilitated discussions around technology implementations that work.
